Who Is the Ideal Candidate for Contoura Vision?

Contoura Vision (topography-guided LASIK) is not “better LASIK for everyone.” It is best understood as LASIK where the laser profile is guided by a high-resolution map of your corneal surface, so microscopic irregularities can be smoothed while your spectacle prescription is treated. The ideal candidate is someone whose measurements justify the added complexity and cost - and who accepts flap-based LASIK trade-offs (dry eye risk profile, flap healing rules) compared with flapless options such as SMILE.

Strong indicators you may be a good Contoura candidate

Meaningful corneal irregularity on topography If your topography shows localised elevation or asymmetry beyond a “textbook smooth” cornea, topography-guided treatment may be discussed. This is especially relevant when patients report ghosting, glare, or quality-of-vision issues that do not fully match a simple sphere/cylinder prescription. Moderate to higher astigmatism (case-dependent) Astigmatism correction benefits from accurate axis and uniform optical zone shaping. Contoura is often selected when the surgeon wants extra personalisation - not because astigmatism alone automatically demands it. Stable prescription As with any LASIK variant, refractive stability (commonly assessed over roughly 1-2 years) matters. Unstable myopia should not be lasered simply because the technology is premium. Adequate corneal thickness and normal shape Contoura does not remove the need for safe residual stromal thickness rules or normal tomography (no keratoconus pattern).

Who may *not* be ideal for Contoura

- Keratoconus or forme fruste patterns on topography - Very thin corneas or marginal safety margins - Severe untreated dry eye or active surface disease - Unrealistic expectations (e.g., guaranteed “super vision” in every lighting condition) - Patients whose primary goal is minimising flap/dry-eye risk - sometimes SMILE or surface ablation is discussed instead

Decision workflow (what a good consultation sounds like)

1. Full refraction + cycloplegic checks when indicated 2. Corneal tomography/topography reviewed in detail 3. Dry eye and lid health screening 4. Compare Contoura vs standard femto-LASIK vs SMILE/PRK with pros/cons - not a single default

Frequently Asked Questions

Not always. If topography is smooth and aberrations are low, standard femto-LASIK may deliver equivalent functional outcomes at lower cost.
