If you have type 1 or type 2 diabetes, blood sugar is only one number that matters. Diabetes remains a leading cause of preventable adult blindness, mainly through diabetic retinopathy — damage to the small vessels of the retina. You can have significant retinal disease and still read 6/6. That is why a specialist eye doctor, and a dilated exam, belong on the same calendar as your HbA1c. In Mumbai, people often treat the physician, the cardiologist and the 'specs shop' as three unrelated visits. For diabetes they are not. The retina is downstream of the same vessels.

What is Diabetic Retinopathy?

Chronically high glucose injures the capillary walls in the retina. In non-proliferative disease those vessels leak fluid and blood, and the macula can swell (diabetic macular oedema) — the usual reason reading vision drops. In proliferative disease the retina, starved of oxygen, grows abnormal new vessels. Those vessels bleed into the vitreous and can pull the retina into a detachment. Both stages can be treated. Waiting until a bleed happens is waiting too long.

The Danger of No Symptoms

Early retinopathy is silent. You will not feel leaking capillaries. Floaters, a blot in central vision, or a sudden red haze usually mean bleeding or oedema that has already declared itself. Annual screening exists precisely because symptoms are a late language. Pregnancy, a sudden improvement in previously very high sugars, kidney disease, and long duration of diabetes all raise risk even if you 'feel fine.'

How Often Do You Need a Dilated Exam?

  • Type 1: usually from five years after diagnosis, then at least yearly — sooner if any retinopathy is found
  • Type 2: dilated exam at diagnosis, then at least yearly
  • If macular oedema or proliferative changes are present: intervals set by the retina specialist, often every few months
  • During pregnancy: more frequent reviews, because retinopathy can accelerate
  • After any sudden floaters, blur, or a dark curtain: same-day, not 'after my next sugar test'

What the Specialist Looks For and How It Is Treated

A dilated exam, OCT (for macular swelling) and, when needed, fluorescein angiography or OCT-angiography map leaking and ischaemic areas. Treatment is staged:
  • Systemic control: sugars, blood pressure and lipids — the foundation; injections cannot outrun uncontrolled diabetes forever
  • Anti-VEGF (and sometimes steroid) injections for macular oedema and some proliferative disease
  • Laser (including pan-retinal photocoagulation) to reduce the drive for new vessels
  • Vitrectomy surgery when there is non-clearing vitreous haemorrhage or a tractional detachment

What You Can Do Between Appointments

Know your HbA1c and blood pressure. Do not skip medicines because vision seems unchanged. Bring a companion to dilated visits. If you use insulin or have fluctuating sugars, tell the ophthalmologist — it affects how aggressively swelling is treated. Smoking makes retinal ischaemia worse. Rapid crash diets and sudden tight control after years of high sugars should be coordinated with your physician, because the retina can paradoxically worsen in that window and needs closer watching.
