Cataracts — the clouding of the eye's natural lens — are a normal part of ageing. If you have been told you have early cataracts, the next question is almost always: when is cataract surgery actually necessary? The answer is not a magic age or a single line on the eye chart. It is how much the cloudy lens is interfering with the life you want to lead, and whether waiting is starting to make surgery harder.
Modern cataract surgery is one of the most successful operations in medicine. That is not a reason to rush it on a mild lens change, and it is not a reason to wait until you cannot cross a Mumbai street at night.
You Don't Always Need Surgery Immediately
In the early stages a cataract may only need a new glasses prescription, brighter reading light, a cap and anti-glare sunglasses, or a little more caution on night drives. If you still work, read, recognise faces and drive with reasonable comfort, your ophthalmologist may simply monitor the lens at regular visits.
What does not work: cataract-dissolving drops, eye exercises, or 'natural' protocols sold online. Once the lens proteins have opacified, the only proven treatment is to remove that lens and replace it with a clear intraocular lens (IOL).
Signs That It's Time for Cataract Surgery
Surgery becomes the right conversation when the clouded lens is costing you independence or safety:
- Night driving feels unsafe because of glare, starbursts or halos around headlights
- Reading, television or phone screens stay hazy even with a new prescription
- Faces are harder to recognise across a room
- Colours look yellowed, dull or washed out — people often notice this only after the first eye is done
- Glasses keep changing and still do not restore useful vision
- You are tripping, missing steps, or avoiding going out after dusk
- The cataract is blocking the view of the retina, so diabetes or glaucoma cannot be monitored properly
- You need refractive or retinal surgery that cannot be planned through a dense lens
Why Waiting Too Long Is Also a Problem
Cataracts usually grow slowly, but they do not stay 'the same.' A hyper-mature, rock-hard lens takes more ultrasound energy to remove, the supporting bag can be more fragile, and the risk of complications rises. In some people — especially with diabetes or a posterior subcapsular cataract — vision can drop quickly over months, not years.
If one eye is already quite poor, delaying the second eye can also leave you without a backup if anything unexpected happens to the better eye. Your surgeon should explain density, not only 'you still have 6/12.'
The Decision Is Yours — With a Clear Work-Up
There is no universal visual-acuity cutoff that forces surgery. A 70-year-old who only reads at home may wait longer than a 55-year-old who still drives to work and operates on screens. The consultation should include:
- A dilated exam to grade the cataract and check the retina and optic nerve
- Biometry to measure the eye for IOL power
- A discussion of monofocal, toric, EDOF and trifocal lens options against your hobbies, night driving and willingness to accept halos
- Systemic issues: diabetes control, blood thinners, and whether both eyes will be done on separate days
- A written sense of what 'success' means for you — glasses for reading, glasses for distance, or as much independence as your cornea and macula allow
What Modern Surgery Looks Like
Most cataract surgery in Mumbai is day-care phacoemulsification: a tiny incision, ultrasound to fragment the cloudy lens, and a foldable IOL placed in the capsular bag. Stitches are usually unnecessary. Femto-laser-assisted cataract surgery is an option in selected eyes; it is not automatically better for every case. Vision is often useful the next day, with drops for a few weeks and a restriction on rubbing, swimming and heavy dust until you are cleared.
